Theodore Anderson was admitted to Southpointe Health and Rehab after an CHF exacerbation and a fall. Mr. Anderson previously was a community ambulator and was independent with all his daily living tasks. He was evaluated by physical and occupational therapy to address his needs and focus on transitioning to his next level of care.        

Upon evaluation, he was extensive assistance for bed mobility and transfers. He was not walking very long distances and required frequent rest breaks. He also required assistance with daily tasks due to being a high fall risk. Mr. Anderson worked very diligently with therapy so he can return to his living arrangement and complete his daily stroll outside. Upon discharge, Mr. Anderson was able to transfer and walk with a rollator with no assistance. He is now able to complete daily tasks and improved his standing balance tremendously. He was happy to finally move into his new home!
