Mark West was admitted to Southpointe Health and Rehab after hospitalization due to a fall at home. Prior to hospitalization, he was living with his sister and brother-in-law, where he used a rolling walker for community mobility and transferred independently. His sister was assisting him with his activities of daily living. He was referred to and evaluated by physical and occupational therapy to address his needs and focus on transitioning to his next level of care. Upon evaluation, he required guarding for bed mobility and transfers. He was walking 60 feet with the rolling walker with guarding due to safety.

Upon discharge, he was able to transfer with independence and was walking 300 feet with a single point cane. He requires supervision with daily tasks. He was happy to move into his new assisted living facility near his family!
