Each resident at Southpointe Healthcare and Rehabilitation benefits from our outstanding nursing and rehabilitation services in a welcoming atmosphere. Southpointe Healthcare and Rehabilitation offers individualized care to include daily living assistance, medication management and specialized levels of care.

We strive to provide each patient with the tools needed to achieve personal independence to the fullest extent possible.



Long-term Care

Long-term care assists those who may benefit from 24-hour nursing availability and assistance with meals, medication management, and other activities of daily living.
The environment at SouthpointeHealthcare and Rehabilitation is designed so residents may live comfortably as part of a community.



Physician Services

SouthpointeHealthcare and Rehabilitation has partnered with local hospital systems to ensure continuity of care. These independent physicians also utilize nurse practitioners so when they are not here, they have licensed and certified advanced practice nurses at the facility to ensure that trained eyes are available and ready to assist in making informed healthcare decisions.



Wound Care

The resident’s interdisciplinary team supports and designs care plans that seek to heal our resident’s wounds under the direction and guidance of an independent physician. Our licensed wound care nurses work seven days a week for residents requiring treatment for complex wounds, including but not limited to post-operative wound sites, diabetic, and decubitus wounds.



Rehabilitation

A team of therapists, under the direction of an independent physiatrist and the support of certified therapy assistants, develop a plan of care for each patient. We offer Physical, Occupational and Speech therapies. Therapy is offered up to three (3) hours a day and up to six (6) days a week depending on the resident’s need and as ordered by their physician.



Short-term Care

Individualized therapy and treatment is provided by skilled clinicians in our motivational therapy gym. Our facility is designed with the very latest therapy assessment, monitoring, and treatment modalities. Our rehabilitation strategy is tailored to the individual. We do this by simulating an environment that resembles the home environment as much as possible.



Pulmonary rehabilitation
Infusion therapy, including TPN
IV, PICC line, Gastric tube care
Lab and pharmacy services
Podiatry, dentist, and optometry services.
